Booking Form

Please complete the following form to book your session. It will ask you to complete a payment and choose a date and time for your chosen session. 

When it is your session date and time please login to Zoom using the form below with the meeting ID and password you received through email.

Time for your Appointment!

After completing Dr. Shaun Arthur's booking form you will receive an email with your Zoom meeting ID and password. When it is time for your appointment, please login to Zoom below using your name, email, meeting ID, and password. We look forward to speaking with you!
